- Submit proof of High School Diploma or equivalent (U.S. accredited or Naces.org evaluated),
and all official transcripts from all colleges and/or universities attended to Enrollment
Services. Electronic transcripts must be submitted to officialtranscripts@msjc.edu. All foreign transcripts must be evaluated by an approved accredited U.S. company
found at . Once completed you will receive a letter stating your program application eligibility
via your Self-Service account.
- Meet with a to review required coursework, develop an educational plan, get an overview of timelines associated with applying to the program, and sign off on your Nursing Transcript Evaluation Request to get your Program Notes.
- All Official Transcripts from other colleges attended must be submitted or already
on file before meeting with a Health and Wellness Pathway counselor so they can sign
off on your Nursing Transcript Evaluation if needed.
- Program Notes: Once all program prerequisites are completed or you are in the last semester of completion you may complete and submit a Nursing Transcript Evaluation to Enrollment Services to get your Program Notes. You must meet with a and have them sign off on your Nursing and Allied Health Evaluation Request Form, then submit this form before the July 1st or December 1st deadline stated on the form electronically to Enrollment Services through their Dropbox on the .
- If you are applying to the LVN Transition program please submit a copy (back and front) of a current and active California Vocational Nursing License with your Nursing Transcript Evaluation Request.
- Once your Evaluation is reviewed and processed by an Evaluator, eligibility notification to apply to the program will be emailed to students and your Program Notes will be found in Self-Service.
- Program Notes can be found in your MSJC Self Service account under Student Planning by viewing the My Progress tab and clicking on Evaluation and Graduation Results under the heading of Program Notes.
- If your Evaluation does not have an expiration date it is only valid for one year.
Evaluation Request Forms submitted after the deadline will not be accepted. No exceptions.
- If Evaluation results confirm eligibility, complete and submit a Nursing Program Application
during the next filing period. Applications are available for download from the ADN
Application page during the application filing period and can be submitted via email
to NursingandAH@msjc.edu during that time (See ADN Application page for more information).

- Complete the Test of Essential Academic Skills (TEAS) exam. We are currently accepting version 7 and will accept your highest score.
